Important Information Before Your First Visit
Please alert the office if you have any medical conditions that may be of concern prior to surgery. These conditions include but are not limited to diabetes, high blood pressure, artificial heart valves and joints, or rheumatic fever. Also, please inform us of any medications you are currently taking, including heart medications, aspirin, or anticoagulant therapy medications.
Patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.
